Claim & Incident Reporting Forms
Claim Reporting
Reporting claims to us or your insurance company in a timely manner is critically important. Often, costly liability claims for accidents or injuries can be averted by immediately reporting these incidents to us. Property insurance claims are best remedied if the damaged or destroyed property can be quickly seen and assessed by a claim representative. Workers compensation claims, involving injuries to your employees, must be reported promptly to comply with state law. And claims involving other types of insurance, especially those involving alleged misconduct by your Club’s officers, allegations of discrimination, etc. should be reported promptly to protect your rights under your Directors & Officers Liability Policy. You do not have to be sued to report a claim. Policies often require you to report any incidents, events or circumstances that are likely to result in a claim under your policy.
Please contact us immediately if you experience an insurance claim or simply wish to discuss a potential problem or matter. We are here to help you.
Incident Reporting
You and your staff should use these forms as much as possible to report all incidents at your club in a timely manner. Incident reports filed at the time accidents or injuries occur, are an important first step in avoiding claims or lawsuits against your swim club.
